Navigating Our Process
We make the transition to home health care as seamless as possible for all involved: patients, families, physicians, and discharge planners.
Referral
Patients can be referred by their primary care physician, healthcare facilities, or self-referral. A physician's order is necessary to start home care, and our staff assists self-referrals in contacting a physician. APEX also helps with additional patient needs, like Durable Medical Equipment, care-related supplies, and diagnostic testing, as requested by the physician or case manager, facilitating a smoother transition.
Benefit Check
Once APEX receives the order, we’ll contact the insurance company and determine if authorization is required and what services are covered. We’ll notify the patient of any co-pays or deductibles and schedule the first appointment.
Plan of Care
Our team of skilled professionals will develop a customized Plan of Care, regularly coordinated and reviewed with the primary care physician for optimal results. Our care team ensures patient and caregiver understanding through ongoing education and assessment, promoting active patient participation in treatment strategies.
